Client Tax Organizer

Checklist of the most common items to bring for me to prepare your tax return
so I can get you the best refund/lowest tax possible!

Income:

 W-2 forms (Wage Income)
1099-DIV (Dividends)
1099-INT (Interest)
1099-G (Unemployment)
1099-R (Pension Plan, Retirement)
1099-SSA (Social Security) 
1099-MISC (Miscellaneous Income)
any other 1099 forms.


Dependants:

 Full Name, Social Security Numbers, Birthdates


Deductions:
bring as many receipts or bills that you can......the more documentation, the better!  

Real Estate Taxes 
 
Mortgage Interest

2nd Mortgage/Home Equity Loan Interest
(credit card interest does not apply)

Charitable Contributions 
(*Note.....for 2008, the IRS is mandating that
CASH contributions will no longer be tax deductible
under any circumstances.  We can use your 2007
contributions, but keep this in mind for the upcoming year,
to use checks or credit cards for all contributions,
even including churches and charities)

Employee Expenses 
(uniform cleaning, work tools, union dues, safety gear, etc.)

Vehicle Usage
(miles driven for work purposes....
excludes commute to and from work)

Business travel expenses

Employment/Job seeking fees

Home Office Expenses

(utilities, communications, etc.)

Investment Expenses
(broker fees, safety deposit box fees)

Education Expenses
(contributions to 529/TAP plan, classes,
college tuition....excludes room/board)

Student Loan Interest Paid

Tax Return Preparation fees
(I have those on file from last year for returning clients)

Medical & Dental 
(*Note....to be able to use medical expenses,
they must total up to be more than 7.5%
of your total income or else you are not permitted
to deduct any medical/dental expenses.
If you are unsure, bring me your bills/receipts and
I'll be glad to figure it out for you and let you 
know if you can use them or not each year)

Medical Expenses
(if you are permitted to use them based on the 7.5% factor) include:
Doctor visits, Copays, Prescription Drugs,
Operations, Insurance Premiums, Labs,
X-Rays, Dental vists, Dentures, Braces,
Glasses & Contact Lenses, Hearing Aids,
Hearing Aid Batteries, Orthopedic Shoes,
Therapy Treatments, Canes/Crutches/Braces,
Wheelchairs, Visiting Nurses/In-Home Care,
Hospital/Emergency Services, Bloodwork.

Casualty Losses
(fire, accident, theft, natural disasters)

Settlement Sheet
(if purchased a home this year)
to compute deductible closing costs

Childcare/Daycare/Eldercare 
(need provider's Name, Address, EIN or SSN, and amount paid)

Self Employed Business Expenses 
(advertising, legal or professional services,
vehicle expenses - mileage OR actual expenses,
office expenses, rent or lease payments,
utilities, telephone, repairs/maintenance,
supplies, taxes/licenses, continuing education,
travel/meals, and others that might pertain to your business)
